Massage Waiver
By signing below, I, the undersigned, acknowledge and agree to the following:
1. Consent to Treatment
I voluntarily consent to receive massage therapy or bodywork from the therapist employed by Namaste in Nature. I understand that massage therapy may involve the manipulation of soft tissues, muscles, and joints and that this treatment may be beneficial for reducing stress, improving circulation, and relieving muscle tension.
2. Disclosure of Health Conditions
I confirm that I have provided accurate information regarding my health history, including any medical conditions, allergies, injuries, or concerns, that may affect my ability to safely receive massage therapy. I will inform the therapist of any changes in my health status before any future treatments.
3. Risk Acknowledgement
I understand that massage therapy is generally safe, but there are inherent risks associated with any physical treatment, including but not limited to soreness, bruising, or irritation. I acknowledge that, although the therapist will take appropriate precautions, there may be risks of injury, and I assume full responsibility for any consequences related to my health or well-being during or after the massage session.
4. Release of Liability
I hereby release Namaste in Nature, its employees, and agents from any and all liability, claims, demands, or causes of action arising from or related to any injury, discomfort, or adverse reaction I may experience during or after the massage therapy session.
5. Confidentiality
I understand that all personal and health information I provide to Namaste in Nature will be kept confidential and will not be shared with any third parties without my explicit consent, except as required by law.
6. Cancellations and Rescheduling
I acknowledge that I am required to provide at least 72 hours notice if I need to cancel or reschedule my appointment. Cancellation with less than 72 hours notice will not be refunded.
7. Payment for Services
I agree to pay for all services rendered at the rates agreed upon before the session.
